We would like to be involved in the discharge planning process before the patient returns home. Getting involved early helps us prepare the home and care routine so the transition from hospital to home is safe and well supported.

Start the conversation before the discharge date
If possible, contact the team while discharge is being planned. Share the expected return-home date and the practical support requested by the hospital team. Together, clarify the home setup, proposed visit times, and who will handle each part of the routine. Confirm the plan when the discharge arrangements are final; a provisional inquiry does not reserve staff or replace hospital instructions.
